Camp Forms

In accordance with New York State Law, ALL CAMPERS must be examined by a New York, New Jersey, Connecticut licensed medical provider in order to ensure that medical requirements for camp entrance and attendance are met in accordance with the New York State Law as outlined on the "Medical and Immunization Requirements for New School Entrants Form” (SH65).

Local campers may use a health record given from your doctor’s office or download the Medical Form.

If you would like to give consent for our Camp Nurse to administer over the counter medication or if your child requires emergency or prescription medication during camp hours please be sure to complete the second page of the Medical From.

If you are an international camper, please bring your current medical information to your New York physician. Click here for a list of local recommended physicians.

Summer Camp Tour

Dwight Summer Day Camp embraces the same philosophy as Dwight School, which customizes the experience for each student based on individual interests and passions. We call this igniting the “spark of genius” in every child. Through a wide range of age-appropriate programs ― including our specialty “spark” camps ― children can explore, discover what they enjoy doing, and pursue that passion while also making friends and having fun.
